3. Are you a member of any church?
6. Will your child have any difficulty communicating in English?
7. Has your child ever been referred for a developmental or academic evaluation?
8. Has your child been sent to a principal, director, or guidance counselor for previous disciplinary problems?
10. Has your child ever been suspended, dismissed, or denied reenrollment from an educational or extended care program for academic, disciplinary, or other reasons?

11. Has your child experienced any of the following academic accommodations?

Tutoring
Modification of grading scale
Special classroom accommodations (oral testing, extended time 1:1 care)

12. Does your child have a history of:

ADD, ADHD (If yes, diagnosis by a licensed psychologist or physician must be received and reviewed prior to rendering a final admission decision.)
Learning Disability (If yes, supporting documents must be received and reviewed prior to rendering a final admission decision.)
Physical Disability (If yes, supporting documents must be received and reviewed prior to rendering a final admission decision.)
13. Has your child ever repeated a grade level?
14. Has your child received a physical, visual, and hearing examination in the last year?

Students with documented academic and/or difficulties, applying for admission to Calvary Road Christian School, must supply all confidential records, present school records, and any additional educational and/or behavioral evaluations before a final admissions decision can be rendered

Sending

Applicants to grades 1 through 6 must complete the “Permission to Release Information” form for each school previously attended. It is accessible under the “Admissions Documents”  sub-menu under “Admissions” on our homepage.
