APPLICATION PROCEDURES FOR NEW STUDENTS

1. Complete and return the following forms in the Application Packet: New Student Application,
Parental Statement of Cooperation, Emergency
Care Information and the Family and Student Questionnaire. These forms must be completed in full and signed before the registration
process can continue. |

2. If a custody agreement is in place and you wish us to follow specific
directions concerning who can pick up your child on designated days, you must include
the court order and a specific written request. If no written request and court
order is enclosed we will allow the child to be removed by either parent or the
emergency contacts (if parents cannot be reached) on any designated school day.

3. When applying for kindergarten-eighth grade, please include a copy
of your child’s report cards and standardized achievement test results (grades first-eighth
only) for the last two years. (Not required for preschool)

4. Bring in your child’s original birth certificate for us to verify.
(Do not make a copy.) We must see the original birth certificate within seven days
of initial attendance. After the seventh day we are required to call local law officials
to report any students whose parents have not shown us legitimate proof of birth.

5. Please note that all preschool, kindergarten and new students in first
through eighth grade must have current (within 90 days) immunization and physical
examination records. It is highly recommended that if you do not have current records
in this area, that you make an appointment with your child’s doctor as soon as possible.
We will accept applications without current medical records, however, the child
may not attend school until these items are secured. The county requires us to have
this information on forms provided by the Commonwealth of Virginia. Copies are available

6. After receiving your completed application (including previous records)
we will begin to process it. Applicants selected for an interview and testing will
be contacted by the school. All students in kindergarten-eighth grade will be tested
and must submit a $40.00 non-refundable testing fee on the test day. All application
and testing information must be completed before an applicant can be considered
for registration. Enrollment decisions will be based on test scores (kindergarten-eighth
only), answers to the Family and Student Questionnaire, and Application. Preferences
will be given to students who have met the previous qualifications and whose parents
are members of Calvary Road Baptist Church and/or already have siblings in our school.

7. Children who are accepted and enroll in CRCS will receive a notification
letter and the forms needed to set up a direct-debit tuition and extended care account.
Per our policy, all accounts must be direct-debit, except for those who wish to
pay for the school tuition in full up front.
